I don't know, but if I had to guess this would be something to do with people who have ileostomy. For people who know what having a stoma is then you know there's an opening in the abdomen, I have it on good authority this is a 'thing', as people have had treatment for chlamydia and gonorrhea infections in them |

I would assume the 4th hole refers to His/Your hole.
So if a lady says she does all 4 holes, anal is a two way street and youre also getting your back doors kicked in, buckle up and enjoy the ride |
