Nothing confusing. Some straight males meet with their Friends to pleasure females. What is confusing about that?

Perhaps I'm mistaken but I've always taken M/M couple to mean two gay or bi men who are in a relationship.

Nope not always. some will advertise as MM as they may be straight friends who only meet on here together with couples or single females or with FF couples. Hope this clarifies it. Only i only ask for what the ladies want to meet with.
