2 dice, ideally different colours or sizes to tell them apart.
Write 6 nawty forfeits for one dice... lick, nibble, kiss etc
Write 6 body parts for the other dice.
Guys names in one bowl, ladies in another, draw one of each, each person throws one dice, body part first, then forfeit, then they throw a dice each again, lowest score has to do the forfeit on the other one!
Or.... just buy dice with all this already printed on them lol!!! (like we did!)
