Depending on your view most restraint/bondage can be seen as the visuals of meat trussed up in a butcher's shop.
However for all things bdsm there is a range of views on the look and there are many who love the aesthetic that different types of bondage brings, whether the sight of someone in rope, steel or chains.
I am more bondage for a purpose than for the look kind of guy. Therefore I see an anal hook as similar to tying someone's hair to another body part or static point. It is an added layer of mental stimulus and of control. The point is not just using it but the context and purpose of using it.
