Orgasm denial isn't usually a build to to orgasm and then give. It's usually a build up then complete stop and walk away - usually with a 'fuck you' attitude, done several times. The person receiving doesn't know that you are going to deny or how many times you are going to deny - it's more 'just because', leading to orgasm when you do.
