Why do cock rings stop the blood from flowing out, but not from flowing in?

Is it that the pressure entering is higher than that flowing out?

this might sound daft but how does it fit? is it just around the shaft at the end,or does it go around the balls too?

Some simply fit around the shaft and some are double and one fits around the shaft while the other fits around the balls...

The cock ring goes round both the base of the shaft and balls keeping the erection stiff until removed. Also it's supposed to increase pleasure as the though of droop receded.

Just don't keep them on for too long if its a shaft type one. More than 20 mins can cause damage due to restricted blood flow...
