There are a lot of answers for this. From my experience being a sissy for my Mistress friend and progressing in strap on play together.
Physical pleasure... most strap on harnesses with a dildo against a base don't give much direct pleasure, but i have seen articles where some do get direct stimulation. It's all about lineing things up to get friction in the right place for her. You can try adding a small bullet vibrator and some harnesses come with this.
