Traditionally it is overtly and over-played feminine dress - the antithesis of masculinity - a signal of someone saying I am no longer male or of someone being forced to declare, I am no longer male. There is a humiliation aspect.
That said, for me it is really about being sub and dressing in feminine ways. Normally overtly feminine, but not with full frills etc. I prefer a sleeker look. But that I say 'I' detracts from the sissiness and if told to wear frills and pink by my Mistress then, of course, I would.

I have those clothes as well of course, this is an excursion into a new realm for us both.
Mistress MJ finds it amusing/a turn on so even if I didn't actually like it myself I'd still be wearing it.
I suppose what we would be looking for is a man or couple who could enjoy that too, although it's not essential.
I'm already at the point where I'm told what to wear & who to meet so it's not any great step for me.
