I treat it like I treat Christmas, it's not the value of the gift that matters.
It's the act of giving something.
If there is a 'big' birthday, I give a small value gift and the rest as cash.

Giving cash is more practical but buying a gift shows that there has been more thought and effort involved, if its a gift that you know they will like, appreciate and use.
If its just panic buying any gift then give cash as it will probably get returned.

Neither.. much as Christmas... don't believe in the hype of having to buy things on a certain day. Pure capitalism.
Rather give a gift when it is really heart felt and personal.. home made preferable or from a small local business and really, if someone knows me well enough they would do the same for me...
