Answering on behalf of the wife:
Its different from an orgasm but happens with it.
It happens only sometimes
It’s a different level of wetness, she’s very wet during sex and a “normal” orgasm
When it does happen, it continues to happen several more times
It smells different from usual orgasm (almonds!!)
It usually occurs in a certain position with fingers being used. ie kneeling up, fingers finding g spot area.
