I thought it originated in a sexual context in the BDSM/Fetish scene to describe folks who did not participate in that way of life, think it just spread.

Now I think it just refers to the sexualy unadventurous

It is, but it's usually termed as plain vanilla isn't it. No toppings, no sauce.
