You can have a surgery to sever the tendon that holds your penis in and "up" because theres an extra 2 inches or so of cock inside your body this alows it exit more fully giving an inch or so more length.

Well, it's a long time ago for me now, so I don't remember specific conversations. But the 'average is inadequate' bit is just a natural progression from 'bigger is better'. It's not so much an explicit thing as an unspoken implication.
