Anyone work in these areas can advise please
I have an elderly relative .
im pretty much her only family in the uk
i do her laundry and shopping. Visit every day .
She is quite capable . Has meals on wheels etc .. but is housebound
she fell ill this week and is now in hospital.
when she comes out . I want to know what kind of home help i can apply for on her behalf . I work a lot . And cant be at beck n call .... i know last time she was in hospital she had visits from district nurses etc
But it stopped / she may have stopped it
What help can she get??

Does she have an allocated social worker?? If not ask the hospital to get in touch with the duty social worker and go from there.
They are responsible for assessing and deciding what help she may or may not need to enable safe discharge.
A district nurse would have only been calling to deal with medical/nursing needs not care needs. |

If u speak to the hospital they can give u all the information u need.
They actually will not discharge her if they thought she wouldn't be getting what she needs at home.
If u cannot be there due to work commitments they will point u in the right direction or even help u find someone who will care for her. |

The hospital will have its own social work team. When patients fit a certain criteria they have to be referred to social worker, physio,ot etc to ensure they are safe for discharge. They can then help to arrange domiciliary care at home if that's what's needed.Hope she gets well and home soon op  |

Now dealing with this as I await my mum's discharge from hospital after a heart attack. Although she has my 86 year old dad at home and her family living close by, they will not release her until they have put a carer in place for part of the day. The hospital are arranging this for us. When I want info I talk to her consultant or the duty sister. She has had first class care, but you need to ask any questions you have, so you are clear what will be happening. Good luck!
