Although he hoped that the next leap would be the leap home, no he never made it home if my memory is correct.
I think his last leap was a bit of soul searching and then he does something to help his hologram friend and ends up leaping for all eternity.

Yeah, an absolutely fantastic show - I'm rewatching it now.
Don't know why but I really fancied Scott Bakula in the episode where he dressed up as a woman
