A) These days mostly to the Chinese.
B) It matters because the interest payments needed to service the debt come from either taxation or more debt, leaving a lot less money to spend on the services that matter. Also (the same as for a personal loan) if you knock your creditors you either cannot get any more credit or (as happened with Greece) the interest rates become so high that most government spending is focused on paying it and austerity bites very deeply.
