Reading the headlines.... xx% of doctors think it would be a good idea to charge people £10 to use A&E.
The intention being to stop people misusing the service and therefore freeing it up for people with a genuine emergency.
Good idea? |

I wouldn't be against actually it but fear it's another step away from free healthcare.
Maybe if you are noticable d*unk you should have to pay something! |

Good point!!
I don't know any more details, just read the headline and thought it would be an interesting discussion. |

So if someone runs over your foot in their car coz they are d*unk (for example) you'd have to pay £10??
Seems flawed to me!
Although there should be something in place to prevent people going there for every tiny little ailment that could be dealt with my a doctor or even sorted over the phone using 111 but what I don't know... |

Yep I think it is. If you're genuinely ill you get the money back.
Before people say it people on benefits /low income, budget for it. The ones I see/know always seem to have cash for cigarettes and/or beer at the wknd?

Seems to me that the A&E crisis is directly related to gp's not giving out of hours or even sensible in hours appointments. My GP currently is taking appointments for march! And calling nhs direct for a splinter results in a recommendation to go to A&E

"wouldn't mind paying if you got £2 back for every hour you wait. You could end up coming out with a tenner profit."
...with that 10 quid, we could just jump on Ryanair and end up in Hungary. Get treated there and come back - then see that our name hasn't even been called yet at the A&E.

As a regular user of A & E I'm going to say I'm for it. The amount of people I've seen with nothing with them is amazing. Last time I was there a lady next to me had had enough of waiting (2 hours) and decided to go, I asked what was the matter, "I think I sprained my ankle". She then walked out perfectly... |

lots of things like this have been thought of, but everything goes against the ethos of the NHS which is free at the point of use.
i already pay for a&e with my taxes, no other money pays for the service.
its not privately funded or subsidised by the EU, but out of all of our wage packets, so to then charge again for it, is a no.
tbh the amount i have used a&e, if they charged me £1000 to use it, but stopped taking the tax to pay for it, i would be massively in profit.
so no, i dont think anyone that pays for the service by working in this country, should be charged for it.
anyone else, on the other hand, is another debate |

What about people who have on going health issues? such as diabetics...
My dad is diabetic, I'd hate to think how many times I've called 999 or driven him to A&E, because he's had a hypo and we've not been able to get his sugar level up or down as case might be.
One occasion I drove him to A&E as didn't want to "bother" the ambulance service with something so trivial, Upon arrival I got told of by A&E staff and informed, this isn't a trivial matter and to call and ambulance that's what they are for...
Also what if it's the day before payday and you don't have £10? or you just simply don't have it...
We should present our NHS number upon arrival to prove we are entitled to the NHS service, that would cut out the "health tourists" |

How is someone supposed to budget for something they may not need or end up needing several times a month.

Personally if I needed a and e I wouldn't care how much they charged to resolve my issue and stop me suffering. Sadly though it shouldn't be that way but too many people misuse the service with alcohol related issues causing massive delays.
One concern would be if it did start to charge could you rightfully complain when misdiagnosed? Kept waiting for long periods of time or just fobbed off? |

I think the reason A&E like you to call an ambulance is that they can act as an early triage to stop a problem getting to hospital. |

Do it as in Germany Switzerland and elsewhere. If you are pissed and out you have to pay the costs in Germany it's Euro 400 and 2nd time it is even more. Would stop "bing drinking" flat free time up at A&E problem solved. |

why?
can d*unk people not have accidents?
what if the accident they had wasnt drink related, ie, sat at home, with a beer, ceiling falls in, is that the fault of the person for drinking?
and thats the problem, its either all or nothing. you cant say 'well if they are fat/thin/tall/short/smoke/drink/gay/straight/black/white/british/foreign then they have to pay, as there will always be circumstance which SHOULD mean someone in the catagory shouldnt have to pay |

never
when people go out for a drink they either have the mindfullness not to drink loads, or they keep up with their mates, get shitfaced and to hell with the consequences.

Having used A&E 3 weeks ago.
I would be very upset at being charged £10 for having a heart attack.
I only went because the pain had got to bad to handle.
Was bad enough being in hospital with all the extra cost involved, never mind £10 charge on top.
Sounds like another idea thought up in the cheap bar in the commons.

I've never looked at it like that... Normally a first response car attends and a few times they have to call an ambulance to take him in....
Hospital is only two miles up the road so never looked at it like that before..... |

I think this would be a very negative step. I can see massive knock on implication when people delay emergency treatment leading to serious medical conditions escalating. Almost all emergency medical conditions become more complicated and therefore ultimatley expensive to deal with if not treated early. |

I agree something needs to be done as the service can't cope as it is, but not sure this is the answer. Too many people use AandE because they can't get an appointment with their own GP. Maybe that needs looking at. As for using it for trivial things that needs addressing so maybe a charge would help.
A and E have saved my life four times this year. £40 seems a fair amount to still be hear. |

How do you define an emergency though?? Everyone has a different point of _iew. I think personally it would benefit the government to have a seperate department in a&e, you go in get triaged by a senior nurse if she deems it to be an emergency she directs you to a specific waiting area to be seen by a&e staff. If she feels it's not an emergency she sends you to another area to see a gp on call at that time. People are using a&e departments as gp surgerys and not the way they are supposed to be used. For instance your child had a temperature you take it to a&e to be looked at turns out it's a common cold just needs paracetamol which hadn't been given. The same goes for the ambulance service, they get slagged of beyond belief in the press etc yet how many calls do they get every day to silly things they shouldn't be called out to??? Some people use the ambulance service as a free taxi ride into hospital as they know they will get treated quicker. It's all wrong! The gmc need to toughen up give the Drs the right to say sorry your not an emergency see your gp or the gp on call. This way waiting times in a&e departments would be sorter and money wouldn't be wasted. |

I agree in thinking the root of a big part of the problem lays in the fact that gp surgeries only open monday to friday 9 til 5. Why not start opening evenings and weekends? I would imagine that would ease a lot of pressure on a & e departments for a start. |

That might be the case in mathematical terms but can you really see THIS government directing savings from A&E to GPs?
Any savings will go straight into Treasury coffers to fund pre-election bribes. |

Excellent idea. Of course as a paying customer if I feel the quality of service is shite I will demand my money back and a free return visit to compensate me
I think some members of the medical profession need to realise that commercialisation of medical services comes with a different set of patient attitudes that the less able or diligent amongst them might find difficult to work with. |

It may be a good idea to some but it is heading for private healthcare, fine if you can afford it but shit if you are on a low income.
We already get people from London using our hospital as its only a 20 min wait, even the 90 min journey there & back is quicker than waiting 5-6 hours in London hospitals. |

Beware of advocating what amounts to a work to rule. The NHS as we know it only functions on the basis of a high degree of flexibility (on both sides).
Can you imagine, for example, a situation where a surgeon's maximum number of permitted/ contracted hours of work expire during an operation which has taken longer than anticipated? |

"10 squid is fcuk all considering the service. i`d pay it
aslong as the money goes back into the NHS"
It will, BUT the admin cost of collecting it and chasing debt for those that turn up without £10 in their pocket will average out at £11.73p. Then in the next government the cost of £11.73 per patient in admin costs will be disputed and presented as too much management so they will sack the administrators and make it a nurses responsibility to collect the money resulting in less nursing hours available for A&E medical work and the total budget for the NHS will be short by £31,659,328.70p and nobody will be able to work out why |

I don't agree with charging upfront as that will put the most vulnerable people at risk. But I do agree that something needs to be done to prevent inappropriate use. Perhaps billing people who are discharged after triage as they don't actually need acute care. Might make people think more carefully before going. |

This National (or is it international?) Health Service that everybody is so proud of is bankrupting the nation.

If u go to A&E here in ireland you have to pay €100 and if you go to out patient clinics its another €100 so i think £10 is very cheap for you to pay in the uk if it was brought in. |

If it is a genuine reason, such as cuts that need stiches or suspected broken bone or just a genuine ccident then I don't think the fee would be charged. It'sd those that are d*unk or taken drugs that should pay. AND dare I say, those pathetic ungrateful people that insult and assault the nurses & doctors should definitely be charged.... by the police. xx |

Having worked in an A&E department in the past....you wouldn't believe the stupid things people attend with! many of which could wait until a GP appointment can be made.
Some are just totally pointless....one lady couldn't stop crying because her cat had died ffs as sad as it was I have no idea what she thought A&E staff could do about
The clue is in the title Accident & Emergency! if it's neither of these then don't attend....simples.

To all those who think you should pay if you have a drink i would ask how much tax is paid per bottle already which goes towards the overall picture? same with ciggies! What about those that get injured doing "dangerous" activities such as running for a bus, cycling down hill, driving a car (then again thats well taxed) etc etc. lighten up lovelies |

I can see where people are coming from about those that don't need to go to A&E and are wasting time, but how do you know what is a waste of time and what isn't without being assessed by a medical professional? Who decides what is and isn't an emergency and whether said emergency is the fault of the injured/ill? When I broke my ankle a few years back I was convinced it was just a sprain and I would be fine if I rested it...it needed to be pinned and plated. If there had been a charge I might not have gone until I couldn't bear the pain any longer and so could have irritated the problem even more.
Would the homeless go without treatment until they had managed to find a tenner to pay their entry? How many people forfeit dental treatment because they can't afford 6mthly checkups and so end up having to visit an emergency dentist for something that would have probably been repairable for a fraction of the cost and next to none of the pain? £10 may not seem much to some, but on the 29th of the month it can seem a hell of a lot to me so can only imagine others will feel the same.
And I agree that upkeep of the system would never be cost effective and it would make life more difficult for A&E staff as they'd have to spend all their shift hearing people complain about paying £10 to visit when they already pay for the services in tax.
Many A&E's do have out of hours GP services so maybe this needs to be utilised better...the assessments for GP or A&E suggested previously makes the most sense to me. Have a medical professional decide which queue you need to be in rather than people risking further harm because they're worried about wasting £10 |

What the doctors think won't get into law. They are just venting their frustration at having people with 5 day old sore toes filling up time when serious cases need more time. I'd be with them tbh. Maybe a better idea would be to make the obvious non-essentials wait for hours or be referred on to the GP out of hours. |

The intention being to stop people misusing the service and therefore freeing it up for people with a genuine emergency.
Good idea?"
If everyone gets a £20 tax rebate why not though some will miss out. In all fairness you only have to go in late on a Saturday night to see how many are misusing it or the system. |

Surely the whole point of the NHS is getting things on the health service for free? Yes it maybe slower, but the patient still gets treated properly, etc? If you want to pay then fair enough as that's what going private is all about. In dont believe though in paying taxes for the NHS if I have to pay again just to be seen. That's just flawed int opinion. |

Maybe they should work out a system where the "users" have to pay. Easier said than done i know or we could do what they do in the states and Australia. Pay nothing to the NHS and pay a lot when we go in or pay a little amount and not so much when we go into hospital. |

How about a system where the NHS pay you NOT to ask for attention?
You present, get assessed and a decision is made about the expected cost of your treatment/ care.
They make you an offer and, if you accept, you go home. |

I would pay £20 for a fantastic service knowing I will not have to wait behind a d*unk or a druggie , they will be outside having spent all funds on chosen substances whilst the rest of us grafters get our treatment if we need it and get on with our lives ,the staff will be happier as no abuse given from sober sane patients . I don't deny that people abuse the system, but those that do will continue to find loopholes/carry on while genuine users are being penalised.
As others have said, too many grey areas. So a person is out on the town having fun and a couple of drinks then gets bottled by some idiot without any provocation. Charge or no charge for getting stitches?

So if you turn up with no money what will they do turn you away. What do you pay national insurance for exactly because you end up paying over the odds for every thing in this country and still get poor quality. Doctors at local gp and local hospital is crap. At least our way they are rubbish. They don't work weekends when most people can only get to doctors because of work purposes so have to use a&e it's just another way to tax the frack out of us |

I'm not convinced this is a real story but, assuming it is, why don't those GPs who support the idea go for it and let patients decide whether or not to stay on their list?
I suspect a number of patients would vote with their feet and the resulting drop in the GP's income would point out the folly of paying for A&E. |

When Aneurin Bevan was asked how he would deal with widespread opposition to the creation of the NHS from GPs, he remarked that he would "stuff their mouths with gold". The coalition has employed the same tactic to dismantle it. |

It's a sad but very realistic thought that the NHS as we've all known and used is a doomed creation.
Drug companies profit- as private companies are supposed to do-, the public misuse it constantly and the nurses, doctors and surgeons by and large are poorly trained and idle, compared to past times, however, extremely well paid. Not to mention widespread thieving, tourist surgery by NHS employees' families, a failure of competent internal regulation of procedures and widespread loss of job satisfaction due to various factors.
Can you honestly see such an organisation continuing in its present form? If it were a private company it would have been declared bankrupt many years ago.
This report about 'what doctors think' has no value. It is a newspaper story made to look like fact and there is no reason to think this levy might ever happen. |

Only those on benefits will get it free for those hard workers get nothing. It's a silly comment to make. They should actually push NHS direct and out if hours doctors harder. I think for non uk born people there should be a small fee. I mean I go visit a doctor in Spain €50 euro fee. Why should it be free here - not very equal... |

Yes REALLY.
The NHS will die.
It cannot go on in its present form, and irrespective of which party is in power at the time. IT WILL DIE.
OK various governments will keep feeding it happy pills (AKA cash) but that will only forestall the inevitable.
Oh and as for the wonderful care in the NHS. Go and ask the relatives of 1200 dead in Staffordshire and those on the Liverpool "care" pathway about the wonderful care.
The NHS is a stinking cesspit run only for the benefit of its employees, and that is only the fault of the left wing bigots who try to play the "care" card when anyone with an ounce of real compassion tries to reform it.
Bottom line is the NHS has been hijacked by the left as a political football, and judging by some of the comments above, they've got more fans than Man Utd.
But as every football supporter knows, the cups are won by the players on the field, not the supporters in the stands. |

Its rather funny that GP's think this is a good idea, but baulk at charging people who have no right to use the NHS as they "don't want to be tax collectors" - what's all that about?
The basic problem with A & E is that staff allow anyone to use the service. Triage should be 1: are you an emergency; yes / no; 2. If the answer is no then you should be sent to a walk in clinic, simples. I spent a few hours in A & E, in agony whilst waiting for them to deal with folks in a far worse condition. One rather impatient woman wanted to be seen because she had a cough, she'd had it for 2 weeks!
There is a direct link between the change to GP contracts and pressure on hospitals increasing. Out of hours GP's who don't know a patient and don't know the medical history are far more likely to admit to A & E.
Charging isn't the answer to this little mess. |

Nonsense Eh.
So which bit would that be?
You think the NHS can survive as it is?
Maybe you think Staffordshire and Liverpool are a right wing plot as is usually the case whenever the left get caught with their pants down.
Maybe I'm wrong about the cesspit bit, but oh no, the abuse and death threats to the whistleblower are fine in left wing circles, are they?
And any one with an ounce of real compassion DOES get shouted down every time the try to reform the vested interests of the left, whether it be in health or education.
Smear and innuendo are the WMD's of the left and I am well used to them, as I've been fighting the scourge of Socialism my whole life.

Bottom line is the NHS has been hijacked by the left as a political football, and judging by some of the comments above, they've got more fans than Man Utd.
But as every football supporter knows, the cups are won by the players on the field, not the supporters in the stands.
Seldom have I read such nonsense (even on Fab).
"
I believe that's popular in Spain and Germany. Italy too, lest anyone feels left* out.

"what if you couldn't pay the tenner?"
Where I am, you don't pay there at the hospital...
They send a bill to your home address, (which everyone supplies on arrival)
And it's 100€ here! |

Not read the whole thread, or much of this (although i did see the headline on the front of a paper the other day).
My thoughts;
* This is likely to be lead by the fact that GPs now 'commission' health services, in particular acute/hospital services. As such, they are now aware of the cost implications of running the service as it comes out of their budgets.
* If GPs were more flexible regarding their surgery hours (or you could actually get through to the reception before 9am) people wouldn't chance going to A&E in no emergency services.
* Shutting walk in centres hasn't helped either. |

What about the vested interest's of the conservative government? A surprising amount of cabinet members and MP's have interests in companies that will benefit from the privitisation of the NHS.

My two pennies worth-
While one laudable aim of this proposal may be to reduce the number of d*unks who clutter & bed-block A&E the damage that such a proposal COULD do is so great as to negate the benefits. I can envisage situations where people neglect/defer going to A&E when in serious danger simply for fear of the cost. Furthermore, it is entirely conceivable that a person with a relatively minor condition won't go to A&E and will then develop a complication that is more expensive to treat.
If one person dies then all the financial savings won't be worth it. There must be more to life than money and politicians should not be in the business of floating reductive populist policies that have no place in humane society- and I say all this as a higher-rate tax payer so I have more cause than many to resent the feckless and stupid abusing services & benefits! |

How much of what's owed is ever actually collected, and at what cost? |

The whole system needs a proper re_iew and change, the world today is very different to when the NHS was established. We as a nation are much less resilient, want to rush to the doctor at the merest sniffle, the population is much bigger and we are all getting older meaning the demands on the health service will continue to increase. But no political party will have the balls to do it, partly because of stupid comments from people saying they will gain personally, and mainly because it is such a hot potato and the right answer is not an easy answer.

Its one third of gp saying they think it will help deter folk who turn up for stuff like colds etc and should the person be deemed as being in real need of hospital treatment anyone like this will geta refund.
That means two thirds still believe in the nhs should as was set up as free at point of service and thats how it should remain.
No doubt there is issue with folk that attend a and e without genuinely needing treatment but other ways have to be found.

The links break the forum rules but a simple Google search of "MP's who will benefit from the privitisation of the NHS" will bring up a list of around around 70 with all but a handful being from the coalition government.

"It's a sad but very realistic thought that the NHS as we've all known and used is a doomed creation.
Really

Last time I looked Merkels centre right party was in charge in Germany, The Partido Popular (centre right) were running Spain, clearing up the socialist PSOE's mess BTW. and Italy after Berlusconi (I really wouldn't know what label to attach to him) has been a political basket case for decades.
The one you (maybe deliberately) missed out was France. There the Socialists are in charge, and the shit that they are in doesn't bear thinking about.
Another point worth mentioning is that Greece was socialist controlled before the EU puppet government was imposed, and Portugal was also a left wing government before their crisis.
Funny isn't it, bankruptcy follows Socialism like a dog follows its master. |

Surely you wouldn't go to A&E anyway, unless you thought it was an emergency. |

The NHS is a wonderful service for those that don't pay for it. It's a disgrace for those that do. I've worked for the NHS for 22 years, and had private health insurance for 20 years. I value my life and health, and that of those I hold dear. |

To the poster who says bankruptcy follows socialism lets have reminder of capitalism and the free market at work.
Black thursday when tories had majority in one day due to incompetence interest rates went up five times in one day killing off thousands of jobs and businesses in one foul stroke.
Fonanciak sector now banks and their unregulated free reign to do as they wish caused the financial melton in 2008 from which we will not fully recover.
Privatisation now energy companies since privatisation have rode roughshod over folk to point of mess we are in cost wise and not regulated enough.
Railways another example of failed privatisation which has only helped shareholdets and not the users and taxpayers.
So whrn criticising socialism et remember the above.
Do not get me wrong socialism has had failures too. |
