I've had several women orgasm from oral while standing but they were fastened to a St Andrews cross. I've had a couple faint from continued licking after they'd orgasmed and fainted (still upright) after the second orgasm. The worse thing that can happen is your legs fail you. I’ve never been hurt. At worse, I need to sit down.
Do it pressed against a wall. When your legs give, lean into the wall. I used to do this pressed against a floor to ceiling hotel window often.

Maybe it is a mental block? Because you know your legs will give way once you do so you kind of stop yourself.
I find making sure I'm pinned against a wall or my arms around their neck/hands in their hair normally helps steady me once I have come. Or even being close to a bed works. I'm normally a bit too fucked by the third/fourth to care if I do fall on the ground.
