Well, it used to mean (B)ig (B)reasted (Woman). But it shifted to (Big) (B)elly (Woman) these days. Curvy, too, used to mean something different; hourglass figure.

To be fair now I'd never heard it referred to as Big Breasted Woman (despite clearly being one) so it made me search for the original use of the term and found this.
'Both the phrase and the abbreviation was coined in 1979 by Carole Shaw when she started the BBW Magazine, that served as fashion and lifestyle journal for big boned women.'
So that would suggest that pre-1979 the term BBW didn't exist as a phrase to describe women at all, and upon the introduction of this magazine was referring to Big Beautiful Women. So in essence, BBW has always meant Big Beautiful Women when referring to women.
