Wallet, phone, keys and reading glasses. But in my pockets not a bag.
If there has to be a fifth thing, a few coins for tips, a coffee or the shopping trolley.

Way more than 5 for me. If I'm going further than the local shops: phone, wallet, keys, portable mobile charger, possibly a plug in charger, book, possibly a second book, shades (irrespective of weather), headphones, possibly spare headphones, water

A good sharp knife
A couple of pieces of baler twine ( you should never leave home without them).
A chocolate bar
A loo roll

I don’t have a man bag , would be easier if I did!
But in my combat trouser pockets I have
Wallet
Phone + charger
2 packs of cigarettes and a Zippo
Knuckle…. Let’s call them knuckle protectors that’s are a safety device or to discourage idiots!
Swiss Army knife (not as a weapon it’s for those little jobs it comes in handy)
There is always a sixth which is my keys.
