Rather concerning that they are now bringing in extra safety procedures AFTER the crash occured. Surely these should have been in place already when people's lives are at stake

Maybe it's only now they realise the safety measures taken weren't sufficient?

same with everything!its like on the roads you see a danger area and think "thats gonna cause an accident" and its not until the accident occurs that action is takin
