This is something that started since PrEP became available. No fear of getting HIV anymore (still PrEP doesn’t guarantee 100% coverage), then people that has been careful for years, always using condoms, saw the opportunity to finally have bb sex. Unfortunately other STD are not covered by PrEP and there is an increase on chlamydia, gonorrhea, syphilis etc. |

Posting certs and trusting fab to be the gatekeeper for bare sex would be a minefield. Who takes responsibility if disease does happen (which it would given time). What about people faking tests? What about the simple verbal or texted results which are common. Legal action would certainly follow after infection after following the fab cert process.
Your idea that "a piece of latex isn't going to help" getting infected abroad is untrue. Condoms massively help with disease control. Pre-prep even married couples were able to prevent hiv transmission between each other in many cases by using condoms.
Even with testing before and after bareback sex there can be a big lag between infection and positive testing. You or your partner could still be a disease / transmission risk even with negative results, if sex happens during the lag between infection and positive test results.

1) Some STD will be on the tests only after 6 weeks. So you have to do at least 2 tests before.
2) You have to do 2 tests after.
3) It’s better to do tests every 3 months if you have more than one partner, cause you can be responsible only for your actions. People lie.
4) Pills don’t give 100% safety from pregnancy. Again you can’t be sure that lady was taking it regularly for a certain period of time.
5) Condoms give the highest protection from STI and pregnancy. 2in1
You can choose non latex condoms, they are thinner. Choose the right size for your girth and sleep peacefully. |

I can only say: it should still be a solved problem because it's a technical coordination problem at heart. The organisation that ensures my dog has a license, is microchipped etc, the human signs a digital statement, it all works smoothly. In principal all you're doing is verifying the person who turns up to an approved testing centre is the same person who hold a website account. That's mostly about a digital certificate of the sort Revenue uses.
As it is, I don't think medical GP notes are sorted out so yes there is a general competency problem letting us down.

So if you have sex with random 4 people in a SEA country, even wearing a condom, and nobody uses the antiviral, then you have a 10% risk of contracting HIV from one of those people on average. Sex with 4x people with condoms sans antivirals is a 2.5% risk.
Then if you had bareback sex with 10 random people in Ireland in 2018, your risk is still 2%.
So that is shocking but the math is there if you want to pick it apart.
In real life of course disease spread is not random, which is why our first priority in my opinion ought to be selection criteria, then physical barrier, then an antiviral and frequent GUM testing is the cherry on top ideal. I want to point out that sex improves health enormously, so good sex is healthier for you, sometimes that is left out of the conversation.
Re: your last comment on time lag and false negatives/positives, I agree entirely. The medical testing part is far more thorny than the computer certificate topic, and as you said already perfection of prevention isn't plausible not least because we don't seem to have a good solution to oral sex and STDs, for culture or technical reasons. I want to say if we actually had a general NCT for sexual health as part of a prophylactic scheme similar to Japan's Ningen Dock for overall body health it'd go pretty far to help though.

Personally, today I have 2 x STIs (Sexually Transmitted Indepentants) and 1 x STD (Sexually Transmitted Dependant) 👀 😅 🫣
I have 2 x adult children (Indepentant, can tale care of themselves the feckers 😂) and 1 x 15 year old (dependant, requires being helped out in life still 🤣)
Hope no one takes this in bad taste, serious issue that sexual health is 🧐
