Copied from an Admin post on another thread, think it answers your question:

If, from browsing your sent mail, you delete the pic using the link which says "remove photo from message", it does just that and the attachment is removed

If when you're attaching a photo and you tick the box that says "Remove photos from message shortly after it's read (improved privacy)" then system automatically removes the attachment once the message has been read (within 24hrs).

I must read the instruction more....I always use the remove photo after viewing button but when I check my sent post after they have read and seen the pics they are always still there, so I tend to just remove them manually myself, I didn't realise it was not automatic
