Last looked... does not always show

On my “last looked” it does not always show those who I know have looked at my profile. Why is that? Is there a setting to hide when looking?

Yes, you can set it so you don't show up as having looked.

I think if they hide their profile after viewing it disappears from the looked at list as well.
